Job Summary
Occupational Therapist needed for BoldAge PACE to plan, organize, and implement OT services for PACE participants as part of an interdisciplinary team. Responsibilities include assessments of functional abilities and home environment, recommending adaptive equipment and home modifications, developing and revising individualized treatment plans, delivering OT interventions across settings (PACE center, clinic, participants’ homes, contracted facilities), educating participants and caregivers on equipment use and home safety, ensuring timely documentation, participating in daily IDT meetings, supervising COTAs, training staff in safe OT practices, and coordinating with home-based support services. Requires current OT license, advanced degree, and 2 years of interdisciplinary team experience (or equivalent training provided for frail elderly population). Monday–Friday, 8:00am–4:30pm, 40 hours/week. Location: Worth, IL, US.
Required Qualifications
- Advanced degree in Occupational Therapy
- Current license to practice Occupational Therapy in the state
- Two years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred (training provided if not present)
- Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care
- Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
- Pass a comprehensive criminal background check that may include federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen
