Job Summary
Perform occupational therapy evaluations and develop individualized plans of care for adult and geriatric patients in skilled nursing and post-acute rehabilitation settings. Deliver treatment focused on functional outcomes, addressing activities of daily living, cognitive function, strength, and positioning while managing discharge planning. Supervise Occupational Therapy Assistants and maintain accurate documentation to support quality care and reimbursement. Collaborate with interdisciplinary team members and facility staff across multiple partner communities, including Eventide Lincoln Care Center, Williamsburg, and Crete, with travel between locations as needed.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license (or eligibility for licensure) in applicable state
Desired Qualifications
- Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
