Occupational Therapist (Beaver Dam)

Job Summary
Empower home health patients to participate fully in daily tasks by conducting skilled evaluations, planning, and treatment to help them re-learn necessary self-care activities. Establish therapy plans that optimize self-care management, rest, social participation, medication, and overall wellbeing while facilitating patient responsibility for their own care. Provide safe, timely, and patient-centered therapeutic interventions in compliance with organizational policies, infection control guidelines, and safety reporting systems. Work flexible schedules determined by patient needs, including weekends and holidays, with a minimum of 0.5 FTE or one shift per month for per-diem roles.
Required Qualifications
- State of Wisconsin Driver's License
- State of Wisconsin Professional licensure for Occupational Therapist
- Bachelor's degree in occupational therapy
- A minimum of one (1) year of recent experience in Occupational Therapy
- CPR/BLS Certification
- Valid Auto Insurance
- Reliable vehicle transportation
- Work schedules determined to meet patient care needs
- Work may be required on weekends and holidays
- Minimum scheduled role is 0.5 FTE
- Minimum work expectation for a per-diem role is one (1) shift/month
Desired Qualifications
- Master's degree in occupational therapy
- Experience and strong knowledge base in home healthy therapy
- Familiarity with variety of equipment, including computer software
- Organization, time management, flexibility, and a positive attitude
