Job Summary
Conduct home health visits in Salem County, NJ, to evaluate patients' functional status, cognitive aspects, and home environments for safety hazards. Develop individualized intervention plans, assess motor and ADL performance, and identify equipment needs to enhance client outcomes. Continually revise care plans with nursing staff and educate families on rehabilitative activities for independent living. Document all observations and interventions using a touch pad tablet. Requires a current New Jersey Occupational Therapist license, one year of recent OT experience, and proficiency with electronic medical records. Base pay ranges from $65-$70 per point.
Required Qualifications
- A current New Jersey Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- A minimum of one year of recent work experience as an Occupational Therapist
