Job Summary
Conduct home visits to adult and geriatric patients in Marion and Florence County, SC, performing thorough evaluations of functional status, muscle function, and cognitive aspects to develop individualized intervention plans. Assess home environments for hazards, identify equipment needs, and revise care plans while collaborating with nursing on multidisciplinary care. Educate patients and family members on rehabilitative care and activities to promote independent living, then accurately document all observations and interventions using a touch pad tablet. Requires a current SC Occupational Therapist license, one year of recent OT experience, and proficiency with electronic medical records.
Required Qualifications
- A current SC Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Organizational skills
- Problem solving skills
- Solid computer skills
