Job Summary
Provide skilled occupational therapy services through home visits within designated territories, performing comprehensive evaluations of patients' functional abilities and developing individualized treatment plans to maximize independence and safety. Assess home environments for safety concerns, recommend modifications or adaptive equipment, and identify assistive devices to improve patient function. Collaborate with nursing and interdisciplinary team members to coordinate comprehensive patient care while educating patients, caregivers, and family members on therapeutic techniques. Continuously assess patient progress and adjust treatment plans as appropriate, accurately documenting all evaluations, interventions, and patient progress using electronic medical records and tablet technology.
Required Qualifications
- Current New Jersey Occupational Therapist (OT) license
- Minimum of one year of recent Occupational Therapy experience
- Graduate of an accredited Occupational Therapy program
- Strong assessment, communication, organizational, and problem-solving skills
- Ability to work independently and effectively manage time
Desired Qualifications
- Prior home health experience preferred but not required
- Electronic medical record (EMR) experience preferred
