Occupational Therapist Assistant Home Health

Deliver therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Design and adapt equipment or living environments, fabricate devices to improve function, and instruct patients and families on home programs and adaptive equipment use. Provide therapeutic treatment and instruction to improve strength, coordination, and range of motion while documenting observations and preparing clinical notes. Participate in care coordination, discharge planning, and community reintegration activities with the interdisciplinary team. This role supports CenterWell Home Health's mission to provide personalized home care for patients managing chronic conditions or recovering from injury.
