Job Summary
Deliver therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Assist in implementing vocational programs, design and adapt equipment, and fabricate devices to improve function and community participation. Provide therapeutic treatment and instruction to patients and families regarding home programs and adaptive equipment use. Report observations to supervisors, document clinical notes, and participate in care coordination and discharge planning activities. This role supports CenterWell Home Health's mission to provide personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, or surgery.
Required Qualifications
- Current and unrestricted OTA licensure
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- TB screening
- Maintain personal vehicle liability insurance in accordance with state minimums or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage
Desired Qualifications
- Minimum six months occupational therapist assistant experience
- Home Health experience
