Occupational Therapist Assistant, Home Health

Provide therapy services planned, delegated, and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment. Assist in implementing vocational and education programs to restore function and promote self-sufficiency. Design or adapt equipment and environments to improve patient independence, fabricate devices to assist with daily activities, and instruct patients and families on home programs and adaptive equipment use. Report observations to supervisors, document clinical notes, and prepare progress reports. Participate in care coordination, discharge planning, and interdisciplinary team meetings to support patient reconditioning and community reintegration. Requires current unrestricted OTA licensure, CPR certification, and a valid driver's license.
