Job Summary
Conduct psychosocial assessments with patients and families to address complex needs such as abuse, neglect, substance use, and end-of-life decisions. Execute discharge plans by coordinating with alternate sites of care and community agencies while documenting all interventions in the electronic medical record. Provide consultation and resources to the healthcare team, maintaining current knowledge of federal and state regulations and relevant community services. Participate in departmental and statewide stakeholder committees as assigned. Requires an Associate's Degree in an accredited therapy assistant program and a valid Michigan driver's license.

- Therapy assistance experience
- LIC-Occupational Therapist Assistant (OTA) - STATE_MI
- CRT-Occupational Therapy Assistant, Certified (COTA) - NB-COT
- LIC-Driver's License - STATE_MI
- CRT-Basic Life Support (BLS) - AHA
- CRT-Basic Life Support (BLS) - ARC
Desired Qualifications
- Experience in neurological injury, TBI, and Spinal Cord injury (based on Spectrum Health entity)
