Job Summary
Provide therapy services planned and supervised by a qualified Occupational Therapist to help patients regain strength, mobility, and independence in their homes. Assist in implementing vocational programs, design or fabricate adaptive equipment, and instruct patients and families on home care and device usage. Document observations in patient records, prepare progress reports, and participate in care coordination and discharge planning with the interdisciplinary team. Maintain required documentation and submit reports as directed by company and facility standards.
Required Qualifications
- Current and unrestricted OTA licensure
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- TB screening eligibility (required for patient-facing role)
Desired Qualifications
- Minimum six months occupational therapist assistant experience
- Home Health experience
