Job Summary
Provide home-based Occupational Therapy assessments and interventions in a multi-disciplinary setting. Conduct client assessments, develop person-centred care plans, write reports for funding bodies (NDIS, DVA, Medicare, HCP Providers), and collaborate with Case Managers, GPs, and other allied health professionals. Travel to clients’ homes, build relationships with referrers, and uphold Lifecare/APM Group values of care, respect, integrity and service excellence.
Required Qualifications
- Occupational Therapy degree
- General AHPRA registration
- Full working rights in Australia
- Clean driver licence and access to own vehicle
- Willing to undergo National Criminal History check
- Ability to work in a multi-disciplinary team and autonomously in the community
