Occupational Therapist - Aliya of Evanston

Evaluate and treat patients in a SNF setting following an individualized plan of care. Develop plans of care, set measurable functional goals, and re-evaluate patient progress to revise goals and reset objectives. Provide education on injuries, progress, and probable outcomes while answering patient concerns. Collaborate with facility staff to ensure excellent care for patients and family members. Supervise Occupational Therapy Assistants. Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes.
