Occupational Therapist (58098)

Provide occupational therapy to patients unable to perform daily tasks due to illness or injury, assessing deficits, establishing goals, and documenting progress. Assist patients with exercises to improve fine motor skills and coordination, while suggesting adaptive equipment like grab bars and shower chairs. Evaluate fall risks and introduce strategies to improve home safety. Educate patients and families on the plan of care, exercises, goals, and self-care. Work as part of a home health team including RNs, LPNs, CNAs, HHAs, PTs, and SLPs focused on the patient's plan of care and personal goals. Maintain a 1:1 therapist-to-patient ratio to impact outcomes. Requires an active Indiana OT license, minimum one year of experience, CPR certification, and knowledge of state and federal home health regulations.
