Job Summary
The Occupational Therapist provides skilled OT services in patients' homes, evaluating functional status and developing individualized treatment plans to maximize independence in ADLs and IADLs, caregiver education, and safety strategies. Collaborates with physicians and interdisciplinary team to coordinate care, educates patients and families, maintains timely documentation, and travels throughout the agency service area to support home-based care.
Required Qualifications
- Graduate of an accredited Occupational Therapy program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy
- Current Connecticut Occupational Therapist license
- Current BLS/CPR certification
- Valid Connecticut driver's license
- Reliable transportation and proof of automobile insurance
- Ability to travel throughout the agency service area
Desired Qualifications
- One (1) year of occupational therapy experience
- Home health experience
- Experience with electronic medical records and point-of-care documentation
- Experience working with geriatric and medically complex patient populations
