Required Qualifications
- Residency or fellowship in Obstetrics and Gynecology completed
- Unrestricted DEA registration
- Board certified or eligible in Obstetrics and Gynecology (acceptable board to hospital)
- Medical Staff Privileges within the sponsoring institution
- Doctorate MD or DO
- Current and unrestricted license to practice medicine in Utah
