Job Summary
Conduct comprehensive nutritional assessments, including dietary history, lifestyle factors, and anthropometric measurements, for patients in a Queens, NY multi-specialty practice. Develop and implement individualized medical nutrition therapy plans for chronic conditions such as diabetes, hypertension, obesity, and gastrointestinal disorders. Provide nutrition counseling and education on healthy eating habits, portion control, and grocery strategies while monitoring progress through follow-up visits and adjusting plans accordingly. Collaborate with physicians and nurse practitioners to ensure integrated care, maintain accurate clinical documentation in electronic health records, and participate in team meetings and quality improvement initiatives. This part-time role requires a bilingual Spanish speaker with a New York State license to deliver evidence-based guidance in a supportive environment focused on improving patient outcomes.
Required Qualifications
- Bachelor's or Master's degree in Nutrition, Dietetics, or a related field from an accredited institution
- Must have a New York State license as a Certified Dietitian/Nutritionist (CDN) or Registered Dietitian (RD/RDN)
- Must be bilingual in Spanish
- Proficient in conducting nutritional assessments
- Proficient in developing meal plans
- Proficient in utilizing electronic health records
- Proficient in providing medical nutrition therapy for various conditions (diabetes, hypertension, obesity, GI disorders, etc.)
