Nutritional Support Coordinator (Remote – US) delivers remote nutrition education, reinforcement, and care coordination for patients with chronic conditions. Handles a panel of 300-350 patients, conducts 50-60+ daily calls, educates on nutrition concepts, collaborates with clinical teams, and ensures HIPAA-compliant documentation. Requires healthcare background (e.g., health coach, medical assistant, LPN) and multi-state licensure or CMA/MA certification, with a 100% remote work setup in a private, HIPAA-compliant home office. The role emphasizes patient outreach, nutrition planning, care coordination, and adherence to care plans within a fast-paced, privacy-regulated environment.
Required Qualifications
- Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, Medical Assistant: Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C) OR Active Licensed Practical Nurse or Licensed Vocational Nurse license: Must have a multi-state compact license. Willing to obtain company-paid state-specific license, if needed.
- Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience.
- 2+ years of Electronic Medical Records (EMR) and computer navigation experience.
- Demonstrated ability to educate patients and simplify complex nutritional concepts.
- HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
- Must be legally authorized to work in the United States.
