Job Summary
Conduct comprehensive in-home assessments with elders, determine eligibility for home-delivered meals, and create care plans. Perform data entry into computer databases, run reports, and communicate with family members, Springwell staff, and community professionals. Requires a Bachelor's degree or equivalent experience, reliable transportation, and a current driver's license. Offers $28.21/hr for a 37.5-hour week with flexible scheduling options including alternate start times and telecommuting. Springwell supports seniors living independently with a village of care and continuous quality improvement.
Required Qualifications
- Bachelor's Degree or higher
- Associate's Degree and at least 3 years' experience working with elders or providing case management services
- completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3 year period
- Experience working with computers, entering data, and running reports
