Job Summary
Receive case referrals, review patient information to determine home care needs, and assign appropriate clinicians. Evaluate case services through conferences and record reviews, guiding clinicians to ensure quality delivery while remaining available during operating hours. Participate in a rotating on-call schedule for seven-day coverage to address patient care needs and ensure continuity. Review clinical diagnoses, medications, and procedures; assist in establishing therapeutic goals and developing plans of care. Attend case conference meetings, conduct quarterly record reviews, and assist in screening, interviewing, and orienting new personnel. Support policy implementation, education programs, and strategic goals while promoting a customer service-oriented approach.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the State
- Graduate of National League for Nursing accredited school of nursing
- Two years management or supervisory experience within the past five years in a home health agency, primary care clinic, or health facility
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation
- Complies with accepted professional standards and practice
- Complies and maintains current CPR certification
- Demonstrates excellent observation, verbal and written communication skills
Desired Qualifications
- BSN degree is preferred
