Job Summary
Licensed Practical Nurse provides general nursing care within Mohawk Valley Health System's Medical Group practices. Collaborates with physicians, providers, registered nurses, and multidisciplinary care team members. Performs tasks within the framework of case finding, health coaching, and supportive care under clinical direction of a licensed health care provider or registered nurse. Maintains standards outlined by NYS Education Law and Nurse Practice Act; participates in quality improvement initiatives and ongoing education; ensures patient safety and effective communication within the healthcare team. Requires LPN licensure in NY and BLS certification; graduate of an accredited LPN program; full-time day shift.
Required Qualifications
- Graduate of an accredited school of licensed practical nursing program
- Current New York State licensure as a Licensed Practical Nurse
- Basic Life Support Certification (BLS) within 6 weeks of hire date
