Job Summary
Coordinate transition plans for nursing home residents moving to community-based living by arranging housing, essential support services, and outreach with families, healthcare teams, and community agencies. Serve as the primary contact for participants while monitoring progress and maintaining accurate documentation in compliance with regulatory standards. Support quality assurance initiatives and regional travel as needed, working within an interdisciplinary team to ensure successful discharge and reintegration. Candidates must live in Philadelphia, Delaware, Montgomery, Chester, or Bucks County to complete home visits.
Required Qualifications
- Associate's or Bachelor's degree in Social Work, Human Services, Nursing, Psychology, Sociology, or a related field
- Bachelor's degree
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check

Desired Qualifications
- 2-year degree
- Previous experience in care coordination, case management, healthcare, or human services
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS)
- Experience working with individuals with disabilities or special needs
- Familiarity with community support systems, housing resources, healthcare, and human services
- Willingness to learn and grow within the organization
