Job Summary
Coordinate care needs to improve member health status and manage chronic conditions in collaboration with physicians. Develop oversight of healthcare support plans for complex conditions such as recurring falls, depression, and dementia. Conduct member medication reviews, manage transitional care from hospital to home, and facilitate advanced care planning. Assist with admission paperwork, health record documentation, and provide health education to members and families. Collaborate with the Care Management team, family caregivers, and treating practitioners while ensuring compliance with safety and regulatory standards. Travel up to 25% within the local market. Requires MSW or LPN with two years of healthcare coordination experience, active CPR certification, and ability to work a flexible schedule.
Required Qualifications
- MSW or LPN
- Valid malpractice insurance policy
- Registered with the applicable state examining board
- Minimum of two (2) years of experience providing healthcare coordination (for MSW)
- Valid Driver's License
- Proof of Insurance/Registration
- Active CPR certification
- Successful results of background check
- Ability to travel up to 25% in the local market
Desired Qualifications
- LPN or RN (preferred)
- Experience working in a health care facility
- Experience working with seniors (preferred)
