Job Summary
Coordinate patient care and ensure all medical needs are met while providing health coaching and education to patients and their families. Utilize electronic health record systems such as eClinicalWorks to document patient information and update medical records. Collaborate with healthcare professionals to develop and implement patient care plans and conduct utilization reviews to ensure appropriate use of healthcare resources. Assist with administrative tasks including scheduling appointments, managing medical supplies, and maintaining accurate documentation.
