Job Summary
Nursing Clinical Coordinator coordinates and manages infusion-nursing care, collaborates with intake, pharmacy, and clinical teams, ensures staffing for patients, schedules visits and billing with home health agencies, maintains patient relationships, tracks orders and recerts, and supports performance improvement activities. Requires active LVN/LPN license in the state and 2+ years in home health or infusion settings; proficient with scheduling, documentation, and communication with internal teams and external agencies; strong organizational and customer-service skills; basic Microsoft Word/Excel proficiency.
Required Qualifications
- Active LVN or LPN license in the state assigned
- 2 years' experience in home health care or home infusion setting or case management preferred
- Knowledge of home health services and intravenous therapies
- Ability to communicate clearly (verbal and written)
- Knowledge of third-party reimbursement preferred
