Job Summary
Coordinate and oversee daily clinical operations for health and supportive services to ensure residents' needs are met. Provide training, supervision, and monitoring of team members delivering direct care, while assessing residents' physical and psychosocial status to initiate individualized service plans. Manage care coordination with physicians, discharge planners, and state agencies, arranging ancillary services as needed. Supervise staff in medication management and personal care, ensuring the philosophy of care is practiced and resident records are documented timely. Communicate regularly with residents, families, and healthcare practitioners to address health concerns and problem-solve with the Director of Nursing on urgent issues. Maintain up-to-date knowledge of care regulations and participate in marketing efforts by representing the residence to outside agencies.
Required Qualifications
- LPN or RN degree
- active license
- Minimum of 2 years working in geriatric nursing
- experience with medication administration

- Current First Aid and CPR card
- Pass a Criminal Background and fingerprint check
- Ability to work a flexible schedule, including weekends, holidays and overtime
