Job Summary
Nursing Assistant/Participant Care Associate (PCA) provides in-home, community, and day health center support to frail elders under RN supervision. Responsibilities include personal care, meal preparation, toileting/incontinence management, medication reminders, light homemaking, transportation to appointments, and care-plan-guided tasks. Maintains accurate documentation of care and progress, responds to emergencies per policy, and adheres to CMS competencies. Requires CPR/BLS, infection control training, TB testing, and a valid Michigan driver’s license with reliable transportation. Training to cover interaction with diverse populations and dementia care; ability to transfer/move patients and perform duties with flexibility to accommodate participant needs.
