Job Summary
Provide direct personal cares and health-related support to individuals in a small residential setting (up to four residents); assist with mobility including transfers and Hoyer lifts; implement and support individual care plans; assist with medication administration and health-related tasks; help with meal preparation, housekeeping, and daily living activities; transport to appointments and community outings as needed; monitor and communicate needs to supervisors; document care electronically; use behavior support strategies and crisis prevention interventions as trained; work collaboratively with supervisors and team members; must be at least 18 with a nursing assistant training/competency program and valid drivers license; responsibilities include promoting dignity, independence, and person-centered care.
Required Qualifications
- Must be at least 18 years old
- High school diploma or GED
- Valid drivers license and proof of auto insurance
- Completion of an approved Nursing Assistant training and competency evaluation program within the last 10 years
- Nursing Assistant certification not required to be current but must not be revoked
