Provide medical care during admission, treatment and discharge; coordinate pharmacy needs and dispense medication with clear med logs; collaborate with extended medical, clinical and UR teams to ensure coordination of care; maintain meticulous medical documentation, tracking and reporting for each client; provide patient education in individual and group settings.
