Job Summary
Nurse Transitions Specialist (RN) in Ohio provides transitional care across in-home visits, bedside/telephonic assessment, and care planning to reduce hospital readmissions. Responsibilities include eligibility assessment for Acute Care Transitions Program, delivering 30-day bedside/home/telephonic visits, program orientation, needs-based care planning, disease-specific education and referrals, medication reconciliation, falls risk and social determinants of health screenings, and coordination with health plans, primary care, and care teams to finalize and implement individualized HCBS service plans with measurable goals. Requires one year of home health care or geriatrics experience, and an Ohio RN license. Strong written/spoken communication and organizational skills are essential.
Required Qualifications
