Nurse Team Leader

Coordinate, implement, and evaluate home care needs for patients recovering from illness, injury, or surgery at home. Provide advanced clinical services including patient assessment, medication management, and patient/family education. Review insurance information, coordinate individual care plans, and ensure findings are shared with family and professional caregivers. Precept new nurses and supervise peers on field visits. Maintain high-quality documentation and communication while managing time effectively. Requires NYS RN license, BSN, minimum three years of home care field experience, and a driver's license.
