Nurse Team Leader

Coordinate, implement, and evaluate home care needs for patients recovering from illness, injury, or surgery in their familiar surroundings. Provide advanced clinical services including patient assessment, medication management, and patient/family education while reviewing insurance information and coordinating individual care plans. Precept new nurses and supervise peers on field visits, ensuring pertinent findings are shared with family and professional caregivers. Requires NYS RN license, minimum three years of homecare field experience, and ability to mentor and coach home care nurses.
