Job Summary
Provides basic nursing care in a home health setting by assessing residents on admission and ongoing basis, communicates changes to the IDT, documents assessments, updates care plans, and implements nursing interventions including medication administration and nursing orders. Requires IV certification (or willing to obtain), CPR/AED certification (or willing to obtain), and strong communication to residents, families, and staff. Maintains SBARR-based timely physician notification, ensures safe and timely interventions, participates in QAPI to address issues, and may be assigned other duties as appropriate. Home health care focus with opportunity for career advancement and benefits; pay range $26.96-$43.95 per hour with regular status for full- and part-time employees.
Required Qualifications
- Licensed Practical Nurse (LPN) license in the state of employment
- Education from an accredited school of nursing resulting in LPN status
- IV Certification required or willing to obtain within six months
- CPR/AED Certification required or willing to obtain within six months
- Must be able to assess basic nursing care needs of geriatric residents
- Excellent communication and organizational skills
- Critical thinking skills
