Nurse (RN or LPN)

Provide direct patient care as an RN or LPN within a multidisciplinary team in a patient-centered medical home. Responsibilities include patient assessment and triage of walk-ins, care coordination with physicians and allied health professionals, coordination of referrals and follow-up, patient education on conditions, treatments, medications, and proper use of equipment, and documentation of care in the EMR. Perform clinical interventions and procedures under clinician direction (e.g., EKGs, dressing changes, suture removal, injections, IV placement/infusions); obtain specimens and order/request diagnostic tests; participate in disease prevention, population health initiatives, and quality/safety improvements. Support and mentorship of other nursing staff; may include Saturday shifts to ensure clinic coverage. Requires AS nursing degree with MA RN or LPN license, BLS certification, and at least 1 year of nursing experience in acute or ambulatory settings; proficiency with EPIC, Outlook, Microsoft Office; strong communication, confidentiality, and teamwork skills.
