Job Summary
RN Case Manager / Compliance Nurse coordinates assessments, care-plan development, supervisory visits, caregiver training, and compliance activities for Franklin County Senior Options, Veterans programs, Long-Term Care Insurance clients, and private-pay clients. Conducts 60-day and 90-day supervisory visits, evaluates client needs, recommends services, provides caregiver orientation and skills validation, performs competency evaluations for caregivers/Home Health Aides, reviews documentation for accuracy and compliance, collaborates with office staff and referral sources, participates in quality improvement and risk management activities, and assists with incident investigations and corrective actions. Flexible, part-time role with opportunities to provide clinical guidance and support to caregivers and staff; requires OhioRN license and valid driver's license.
Required Qualifications
- Current Ohio RN License in good standing
- Minimum of 2 years nursing experience preferred
- Home care, hospice, senior care, or case management experience preferred
- Knowledge of Franklin County Senior Options and Veterans programs preferred
- Strong assessment, documentation, and communication skills
- Ability to work independently and manage multiple priorities
- Reliable transportation, valid driver's license, and proof of insurance
- Proficiency with electronic documentation systems
