Job Summary
Coordinate patient transitions from acute to post-acute care by conducting assessments, directing screenings, and providing rapid responses to referrals. Collaborate with physicians, stakeholders, and multidisciplinary teams to develop individualized discharge plans, self-management education protocols, and disease management strategies. Educate patients and caregivers on health promotion, wellness, and home care skills while evaluating progress and resolving barriers to self-management. Document assessments, progress notes, and flow sheets thoroughly. Participate in performance improvement activities and community outreach presentations to optimize clinical outcomes and resource utilization.
Required Qualifications
- Graduate of an accredited RN program
- Must be able to communicate effectively in English, both verbally and in writing
- CPR Certification
- Registered with the Board of Nursing for the State of Texas
- Must have good computer skills
- Ability to distinguish between numbers and symbols
- Corrected vision and hearing to normal or near normal range
- Primarily works the day shift tour of duty; additional hours may be asked to work other hours as needed, including weekends
- May be expected to lift over twenty five (25) pounds
- Hand-eye coordination and manual dexterity with substantial movement of wrist and/or fingers
- Walking and climbing to accomplish tasks
- Extended periods of sitting and/or walking depending on specific duties
