Nurse (RN) – Capacity Coordination Center, Rotation

Job Summary
Coordinate patient transfers and manage the transfer queue by assessing clinical needs, reviewing medical records, and directing communications between referring providers and Dartmouth-Hitchcock Health staff. Facilitate urgent, emergent, and inter-facility placements for critical care, neonatal, pediatric, and adult patients while prioritizing activities based on EMTALA, active cancer care, and readmission criteria. Collaborate with the On-Call Medical Director and charge nurses in daily capacity huddles to develop mitigation strategies, correct census errors, and ensure accurate patient placement systems data. Work in a 12-hour D/N rotation (24 hours/week) as part of a multi-disciplinary team to maximize safe patient intake and bed flow management.
Required Qualifications
- Graduate from an accredited Nursing Program
- Bachelor of Science Degree in Nursing (BSN)
- 3 years of Critical Care experience
- Leadership experience
- Previous computer experience (Word, Windows, and Excel)
- Proven organizational and multi-tasking abilities
- Strong customer service skills
- Excellent phone etiquette
- Licensed Registered Nurse with NH eligibility
- BLS or ACLS certification
