Job Summary
Nurse Practitioner to provide in-home primary care services in the Athens area as part of a physician-group-led care team. Responsibilities include evaluating high-risk patients, developing individualized care plans, coordinating with interdisciplinary team members, communicating with case managers and specialists, participating in team meetings, ensuring regulatory compliance, documenting care activities, educating patients and families, and supporting the patient in self-management and preventive care. The role involves visits across homes, facilities, telehealth, clinics, and specialties, with full-time salary-based compensation and bonus opportunities. Requires NP/PA/Physician licensure in the employment state, prior experience in home health, telehealth, or related settings, strong communication and leadership skills, and the ability to travel between offices and facilities.
Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required
- License must be in good standing
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting
- Strong communication, collaboration, and interpersonal skills
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities
- Commitment to delivering compassionate, patient-centered care
- Strong organizational and time management skills
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
