Job Summary
Evaluate high-risk patients with complex medical needs and develop individualized care plans. Coordinate care activities with the interdisciplinary team to ensure continuity and optimal patient outcomes. Communicate regularly with case managers, specialists, pharmacists, and social workers to facilitate seamless patient management. Document care management accurately and provide patients with education on self-management and preventive care. Monitor quality measures and implement corrective actions as needed. Participate in interdisciplinary team meetings to discuss patient progress and treatment plans. Adhere to all relevant healthcare regulations, including HIPAA and Medicare guidelines.
Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required
- License must be in good standing
- Demonstrated experience in providing patient care
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- preferably in a home health, clinic, facility, telehealth, or specialty care setting
