Job Summary
Nurse Practitioner role providing home-based and facility/clinic care in the Columbia, SC area. Responsibilities include evaluating high-risk patients, developing individualized care plans, coordinating care with interdisciplinary teams, and delivering care management to improve patient outcomes. Duties span visits in designated service area (homes, facilities, telehealth, clinics, and virtual visits) with 12-hour shifts (7 days on/7 days off) or 8-hour daytime shifts (Mon-Fri, 8:00am–5:00pm). Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required.
- License must be in good standing.
- Demonstrated experience in providing patient care in home health, clinic, facility, telehealth, or specialty care setting.
- Strong communication, collaboration, and interpersonal skills.
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities.
- Must be able to work flexible hours and travel between offices/facilities.
- Must be a licensed driver with an automobile insured and in good working order.
