Job Summary
Nurse Practitioner – Wound Care Team Leader responsible for leading the wound care program in-home, providing expert wound care assessments and management, mentoring wound care nurses, coordinating with physicians and interdisciplinary teams, monitoring clinical quality and compliance, and driving continuous quality improvement. Role requires NP licensure, wound care credentials (preferred WOCN), 3–5 years wound care experience (in-home/post-acute preferred), BLS, strong communication and EHR skills, and reliable transportation for in-home visits. Expect leadership responsibilities, education for patients and caregivers, and a focus on patient-centered outcomes.
Required Qualifications
- Current, unrestricted Nurse Practitioner licensure in the state(s) of practice
- Wound care certification such as WOCN or equivalent recognized wound care credential preferred
- Minimum 3-5 years of clinical experience with a strong background in wound care, preferably including in-home or post-acute care settings
- Demonstrated leadership experience in clinical settings or wound care programs
- BLS certification or higher
- Proficiency with electronic health records and digital wound care management tools
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Valid driver’s license and reliable transportation for in-home visits
- Strong commitment to patient-centered care and quality improvement
