Job Summary
Nurse Practitioner – Wound Care Team Leader leading in-home wound management program delivery. Oversees clinical outcomes, mentors wound care nurses, collaborates with physicians and case managers, monitors quality and regulatory compliance, and drives continuous quality improvement in in-home care. Roles include education for patients/caregivers, leadership to promote best practices, and acting as a clinical resource for wound care with strong emphasis on teamwork, accountability, and clinical excellence.
Required Qualifications
- Current, unrestricted Nurse Practitioner licensure in the state(s) of practice
- Wound care certification such as WOCN or equivalent recognized wound care credential preferred
- Minimum 3-5 years of clinical experience with a strong background in wound care, preferably including in-home or post-acute care settings
- Demonstrated leadership experience in clinical settings or wound care programs
- BLS certification or higher
- Proficiency with electronic health records and digital wound care management tools
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Valid driver’s license and reliable transportation for in-home visits
- Strong commitment to patient-centered care and quality improvement
