Nurse Practitioner - Wound Care (In-Home Care)

Job Summary

Conduct expert wound care assessments, treatment planning, and management in the home setting following evidence-based guidelines and company protocols. Lead and mentor wound care nurses within the assigned market to promote best practices and enhance clinical outcomes. Collaborate with interdisciplinary teams including physicians, case managers, and other healthcare professionals to coordinate patient-centered care. Monitor clinical quality, compliance, and performance metrics to ensure adherence to regulatory and company standards. Serve as a clinical resource and subject matter expert for wound care within the market while participating in continuous quality improvement activities, product evaluations, and new treatment protocol development. Engage with patients and caregivers to provide education, support, and ensure satisfaction with care delivery.

Required Qualifications

  • Current, unrestricted Nurse Practitioner licensure in the state(s) of practice
  • Minimum 3-5 years of clinical experience with a strong background in wound care
  • Demonstrated leadership experience in clinical settings or wound care programs
  • BLS certification or higher
  • Proficiency with electronic health records and digital wound care management tools
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid driver's license and reliable transportation for in-home visits
  • Strong commitment to patient-centered care and quality improvement

Desired Qualifications

  • Wound care certification such as WOCN or equivalent recognized wound care credential
