Nurse Practitioner - Wound Care

Job Summary
Conduct expert wound care assessments, treatment planning, and management in the home setting while leading and mentoring wound care nurses within the assigned market. Collaborate with interdisciplinary teams including physicians and case managers to coordinate patient-centered care, monitor clinical quality and performance metrics, and serve as a clinical resource for new treatment protocol development. Engage with patients and caregivers to provide education and support, fostering a culture of teamwork and accountability. Requires current unrestricted Nurse Practitioner licensure, wound care certification, and 3-5 years of clinical experience with a valid driver's license. Mileage reimbursement and comprehensive health benefits are provided.
Required Qualifications
- Current, unrestricted Nurse Practitioner licensure in the state(s) of practice
- Minimum 3-5 years of clinical experience with a strong background in wound care
- Demonstrated leadership experience in clinical settings or wound care programs
- BLS certification or higher
- Proficiency with electronic health records and digital wound care management tools
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Valid driver's license and reliable transportation for in-home visits
- Strong commitment to patient-centered care and quality improvement
Desired Qualifications
- Wound care certification such as WOCN or equivalent recognized wound care credential
