Job Summary
Oversee clinical care and treatment of patients in their homes, providing wound care expertise after grafting while consulting with attending and hospice physicians. Direct the interdisciplinary team to ensure quality palliative and end-of-life care, completing face-to-face encounters before and after the third benefit period. Educate patients and families on disease processes, self-care techniques, and resource acquisition, while actively participating in interdisciplinary group meetings to evaluate patient outcomes. Perform visits based on individualized plans of care, adhering to standard precautions and HIPAA policies, and provide on-call services as assigned. Requires two years of advanced practice experience with a preference for hospice or home health settings.
Required Qualifications
- Graduate from a Baccalaureate School of Nursing
- Nurse Practitioner (NP) focused Master's or Doctoral Nursing Program
- Current state license as a Registered Nurse
- Current state license as an Advanced Practice Nurse
- Current driver's license
- Two years' experience as an Advance Practice Nurse
- in-home Wound Care experience
- Experience working with ordering and applying wound care products after grafting
- Comprehensive knowledge of pharmacology
- Comprehensive knowledge of palliative care
- Comprehensive knowledge of end-of-life
- Good interpersonal skills
- Reliable transportation
- Valid auto liability insurance
- Ability to work a flexible schedule
- Ability to travel locally
- Ability to lift up to 50lbs
- Ability to move patients
- Ability to make quick decisions
- Ability to acquire resources
- Ability to meet patient and family individualized psychosocial needs
- Ability to complete face-to-face encounters with patients
- Ability to provide clinical assessment results to hospice physician
- Ability to use standard precautions
- Ability to adhere to hospice policies regarding protected health information
- Ability to keep up with and maintain education/knowledge in phrenology and palliative medicine
- Ability to provide on-call services to patients/families
