Job Summary
Conduct comprehensive primary care visits and physical exams in the comfort of a patient's home, creating longitudinal care plans for high-risk patients to prevent ER visits. Manage a dedicated group of seniors with full support from an interdisciplinary team, addressing social determinants of health while solving problems autonomously. Deliver field-based care that treats the whole person, ensuring seamless and safe care transitions. Requires an active, unencumbered NP license with Board Certification, a Master's degree, and a valid driver's license for local travel.
Required Qualifications
- Master's Degree (NP)
- Active, unencumbered NP license
- Board Certification (ANCC or AANP)
- Valid driver's license
- Vehicle for local field-based travel
- Legally authorized to work in the United States without the need for employer sponsorship now or in the future
- Successful completion of a pre-employment drug screening
- Compliance with the company's Drug-Free Workplace Policy
- Successful completion of a background check
- Satisfactory results of monthly FACIS (Fraud and Abuse Control Information System) checks
