Job Summary
Under the supervision of the Medical Director, provide primary care to PACE participants by performing comprehensive history and physical assessments, semi-annual reassessments, and developing individualized plans of care. Evaluate and treat individuals with chronic and acute illnesses, managing care in clinic settings or participants' homes before escalating to higher levels of care. Coordinate with the Interdisciplinary Team to oversee participants residing in Skilled Nursing Facilities and provide education to participants and caregivers. Participate in the on-call rotation and perform other duties as required. This role supports the mission of helping individuals age 55 and older live safely and independently within Western Colorado.
Required Qualifications
- Graduate of a school of professional nursing
- Licensed as Registered Nurse (R.N.) in the state of employment
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- Prescriptive authority required within one year of hire
- Valid driver's license
- Proof of insurance
- Means of transportation
- Minimum of one year of experience in working with the frail and elderly population
- Participation in the on-call rotation
Desired Qualifications
- Certified as a Geriatric Nurse Practitioner (G.N.P.)
- Two (2) years of experience as a nurse practitioner in a geriatric setting
- Working knowledge of the PACE regulations
