Job Summary
Nurse Practitioner to join a primary care team delivering telehealth-based urology-focused care. Responsibilities include evaluating high-risk patients, developing and coordinating individualized care plans, collaborating with an interdisciplinary team, documenting care activities for regulatory compliance, educating patients and families on treatment options and preventive care, and supporting care across home, facility, telehealth, and clinic settings. Requires NP/PA/Physician with license in GA/State of employment; strong communication, leadership, and multitasking abilities; willingness to travel between offices; HIPAA/Medicare knowledge; and capacity to provide compassionate, patient-centered care. Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
- Strong communication, collaboration, and interpersonal skills.
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities.
- Commitment to patient-centered care and excellence in healthcare delivery.
- Strong organizational and time management skills.
- Ability to read and communicate effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Ability to manage and demonstrate effective leadership skills.
