Job Summary
Nurse Practitioner to provide clinic-based care primarily in the Poinciana Clinic, collaborating with Upperline’s Physician team to develop and implement complex plans of care for adults with chronic conditions. Responsibilities include performing history and physical exams, ordering diagnostics, developing treatment plans, educating patients and families on medications, diet, nutrition, and crisis prevention, coordinating with the interdisciplinary team, maintaining medical records per state regulations, and participating in weekly team meetings. Requires a Master’s degree in nursing, board certification as an Adult/Adult-Geriatric/Family NP, current Florida NP license, BLS, and 2+ years of Primary Care/Internal Medicine NP experience; DEA registration required or eligible. On-site role with a regular weekday schedule in Poinciana, FL, with telehealth and home-based care elements part of the scope. Compensation will be commensurate with region and experience. Job Type: Full-time.
Required Qualifications
- Master’s degree in nursing
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner
- Current Florida State Registered Nurse & Nurse Practitioner license
- Basic Life Support (BLS) certification
- 2+ years nurse practitioner experience in Primary Care/Internal Medicine
- Possession of DEA registration or eligibility
- Must be comfortable providing care in patients homes
- Experience with EMR - AthenaHealth EMR highly preferred
- Excellent communication skills
- Ability to prescribe medication as permitted by state Board of Nursing
