Job Summary
Develop and implement clinical plans of care for adult patients facing chronic conditions such as diabetes, COPD, and CHF in clinic or home settings. Gather patient history, perform physical exams, and conduct diagnostics while prescribing medication as permitted by state regulations. Collaborate with the interdisciplinary team, including physicians and care navigators, to coordinate optimal care and communicate medical information regarding medication usage, diet, and crisis prevention. Provide telephonic support for escalated triage calls and participate in weekly team meetings and continuing education sessions. Maintain accurate medical documentation consistent with risk adjustment standards and state policies.
Required Qualifications
- Master's degree in nursing
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner
- Current State Registered Nurse & Nurse Practitioner license
- A minimum of one (1) years Nurse Practitioner experience in primary care / internal medicine
- Value-based care experience
- Possession of DEA registration or eligibility
- Basic Life Support (BLS) certification
- Must be comfortable providing care in patients homes
- A reliable automobile, valid driver's license and minimum state required liability auto insurance
- Adaptability to thrive is a fast-paced growth-forward environment
- Experience working in an EMR
- Proficiency using Microsoft Office products
- Excellent oral and written communication skills
- Must be available for regular weekday schedule: M-F, 8-5
Desired Qualifications
- Previously worked in a value-based care environment
- Experience with primary care / internal medicine, geriatric care and/or home-based care, palliative or hospice care
- Previous experience managing, supervising, or coaching clinicians
- Experience with AthenaHealth EMR
